The OPL Team Registration Form is required for each team to fill out.  It is a four part form:  1) asking for the contact info and other information about your team.   2) Signing that your team agrees to follow the rules of the OPL  3) Agreeing that your coaching staff will follow the Ohio Return to Play Law (Concussion Law) of the State of Ohio  4) Agreeing that your coaching staff and team will be in compliance with the Lindsay's Law  5)  Agreeing that your coaching staff is in compliance with the Safe Sport Act.
